ID Replacement Card Fee

The Identification Card (ID) is your official university identification. You will need to carry it with you at all times on campus and be presented upon request to university officials. The card is non-transferable. By using this card, you agree to all terms and conditions in effect at the time of its use. You are responsible for bringing your ID back with you each semester. Your ID is the property of Shenandoah University and will be given to the Enrollment Management & Student Success office upon request, withdrawal, or graduation from the university.

To replace a lost or stolen ID Card, please complete the below form. 

Replacement ID Cards can be picked up in the Campus Services Office on the second floor of Cooley Hall (Room 208). 

Office Hours: Monday-Friday, 9am-5pm
**ID replacements will not be processed outside of office hours**
